  • Choosing a Representative: You can designate someone to act on your behalf regarding your PHI by providing written authorization.
  • Filing a Complaint: If you believe your privacy rights have been violated, you can file a complaint with our Compliance Officer (details below) or the Department of Health and Human Services.
